Introduction: Children with epilepsy and neurologic disorders who fail to thrive have poor outcomes and a longer hospitalization. There is uncertainty around the interpretation and management of severe in-hospital seizures. We aimed to compare the clinical characteristics, outcomes, and risk factors associated with neurologic conditions that fail to thrive and idiopathic seizures that fail to thrive. Methods: We conducted a retrospective analysis of medical records from a tertiary children’s hospital. We defined patients as failing to thrive if they met our established definition of failed to thrive (FtT). We included patients with neurologic conditions (including hypoxic ischemic encephalopathy) and idiopathic seizures who failed to thrive and patients with idiopathic seizures who were not considered to be failing to thrive. We compared the clinical characteristics, outcomes, and risk factors associated with neurologic conditions that failed to thrive to those of idiopathic seizures that failed to thrive. Results: One hundred and four patients met criteria for FtT, including 19 patients with idiopathic seizures (18.3%), and 85 patients with neurologic conditions (81.7%). There were no significant differences in age (p = 0.6), gender (p = 0.85), or admission Glasgow Coma Scale (GCS) (p = 0.19) between the neurologic conditions that failed to thrive group and the idiopathic seizures group.
